自從體外沖擊波碎石術ESWL（Extracorporeal Shockwave Lithotripsy）上世紀80年代初開始應用于腎結石的治療以來，體外沖擊波的治療指征已有了很大的擴展，體外沖擊波的技術性能也有了相當大的改善。在疼痛康復領域，上世紀80年代末發現體外沖擊波可用于假關節和末端病的治療，比如說骨折延遲愈合、上髁炎、肌腱鈣化和足底筋膜炎（跟刺）等。由此體外沖擊波從單一的“體外沖擊波碎石ESWL”演變產生出了新的治療方法“體外沖擊波治療ESWT”（Extracorporeal Shockwave Therapy）。

沖擊波是一種通過物理學介質傳導的機械性脈沖震波，是壓力急劇變化的產物。在短短的幾納秒內產生很高的和瞬間下降的壓力幅度，這是沖擊波獨有的特性。壓力急劇變化產生的沖擊波具有很強的壓應力和張應力，所以能夠穿透任何彈性介質，如液體和軟組織。早先的體外沖擊波治療仍采用傳統的泌尿科碎石術ESWL的聚焦型（壓電式、液電式或電磁式）高能量硬式沖擊波波源，設備依舊龐大、昂貴和復雜，對治療的部位需要X線或超聲定位，機器能量大，需要局部麻醉，治療副作用多且明顯，患者恢復期較長，臨床也難以得到普及。
